Clinical Dashboards using HIE Data
Overview
The Data Rivr team developed a set of Clinical Dashboards to allow providers to view their patients' data to support care and chronic disease management.
Valuable Data
Using the HIE data as a base, the Data Rivr team setup services to receive ongoing data to feed to the Clinical Dashboards. The new/updated data was processed and run through algorithms to assess patient clinical status for the following registries:
- Hypertension (Uncontrolled and Undiagnosed)
- Diabetes (Undiagnosed and Prediabetes)
- Osteoarthritis
- Rheumatoid arthritis
- Hospital and ED readmissions
- Quality Measures
Integrations
As part of this project, the Data Rivr team utilized the following healthcare technologies:
- HL7 and FHIR standards
- CCDa document exchange
- Integrations with EHRs
